Diabetes Coalitions & Support Groups – The Breathitt County Diabetes Coalition DETAILS: Meets at The Breathitt County Extension Office, 1155 Main St., Jackson, KY 41339. Diabetes Self-Management Education & Support Classes (DSMES) – Kentucky Homeplace (Dept. for Aging & Independent Living) DETAILS: Stanford’s Chronic Disease Self-Management Program (CDSMP). Classes are designed to be taught by trained non-licensed professionals who are living with a chronic disease. Classes meet for 2.5 hours a week for 6 weeks. Call to Register. – The Kentucky River Area Agency on Aging & Independent Living, at The Breathitt County Senior Citizens Center (Call for class location) DETAILS: Stanford’s Chronic Disease Self-Management Program (CDSMP). Classes are no more than 1 hour in length. Classes are designed to be taught by trained non-licensed professionals who are living with a chronic disease.
